Smiles from Ear to Hear recognizes that all personal information is private and confidential.  We are committed to protecting the privacy and confidentiality of our patient information.  It is the intention of Smiles from Ear to Hear, by way of this Privacy Policy to comply with applicable privacy legislation.  This Privacy Policy briefly describes how we collect, hold, use and whenever necessary, disclose your personal information.

Ms. Demetra T. Valente acts as the Privacy Officer.  All staff members who come into contact with your personal information are trained in the appropriate uses and protection of your information.

Our clinic will ensure that:

  • Only necessary information is collected about you;
  • We only share your information with your consent;
  • Storage, retention and destruction of your personal information complies with existing legislation, and privacy protection protocols;
  • Our privacy protocols comply with privacy legislation, standards of the College of Denturists of Ontario (CDO), and standards of the Association of Hearing Instrument Practitioners (AHIP), as applicable.

This clinic will collect, use and/or disclose personal information about you for the following purposes:

  • To deliver safe and efficient patient care;
  • To identify and to ensure continuous high quality service;
  • To assess your health needs;
  • To provide health care;
  • To advise you of treatment options;
  • To establish and maintain communication with you;
  • To communicate with other health-care providers and including specialists;
  • To complete and submit claims for third-party adjudication and payment;
  • To comply with legal and regulatory requirements, including the delivery of patients’ charts and records to the CDO in a timely manner, as required, according to the provisions of the Regulated Health Professions Act and AHIP policies and procedures;
  • To deliver your charts and records to the insurance carrier to enable  the insurance company to assess liability and quantify damages, if any;
  • To prepare materials for the Health Professions Appeal and Review Board (HPARB);
  • To comply generally with the law;

Our clinic will not supply your insurer with your medical history without your express consent.  When unusual requests are received, we will contact you for permission to release such information. You may withdraw your consent for use or disclosure of your personal information at any time by providing written notification.

By signing the patient information form, you are also confirming that we have provided you with a copy of the privacy policy for Smiles from Ear to Hear.  If a new purpose arises for the use and/or disclosure of your personal information, we will seek your approval in advance.

You are also acknowledging that the information may be accessed by regulatory authorities under the terms of the Regulated Health Professions Act (RHPA) for the purposes of the College of Denturists of Ontario fulfilling its mandate under the RHPA, and for the defense of a legal issue including the Association of Hearing Instrument Practitioners procedure and policy requirements.
